PREPARE FOR YOUR APPOINTMENTS

At My Family Doctor, we want to help make your upcoming visit a great care experience. Here are some ideas to help prepare for your visit:

 

Before My Visit
 

What to bring

 

  • Your medicine bottles and containers or list of all current medicines - prescription medicines, over-the-counter (OTC) medicines, herbal/naturopathic remedies, and vitamins/supplements.

  • Any home-monitoring information (for example, blood pressure or blood sugar readings).

  • If you're being treated by any other health care providers, please bring the list and contact information of your care team. This will help us better coordinate your care.

  • A pen and paper to take notes

  • A family member or close friend (primary support) if you wish

 
Please Consider
 
  • Arriving just 15 minutes early for your scheduled visit to check-in and allow nursing staff to gather your health information

  • Routine and annual appointments are generally scheduled a number of weeks in advance.

  • Urgent needs can be accommodated within a day or two, and true emergencies are seen immediately.

  • If an urgent or emergency visit is necessary, you may be seen by someone other than your usual health care provider.


During My Visit
 
  • Ask questions and be involved in your care!

  • Provide your questions and concerns to your care team and doctor right away

  • Ask questions if you don't understand what is being discussed

 

After My Visit
 
  • Follow the plan that you and your doctor agreed upon

  • If you scheduled a follow-up visit with your doctor/Nurse Practitioner (NP)/Physician Assistant (PA) or a referral visit to another doctor, put these dates on your calendar right away

If you need to cancel your appointment, we ask you to do so whenever possible with at least 24 hours notice.  In return, we work very hard at running on time too.

 

Sometimes our practice has urgent or emergency appointments come in, and even though we leave time in our schedules to accommodate such problems, we will sometimes be unavoidably late. We appreciate your patience and understanding when this happens.  We try as best we can to minimize such lateness. We reserve the right to charge for missed appointments without 24 hours notice.
